2013-14 STUDENT INSTRUCTIONS
FOR INITIAL ATHLETIC PARTICIPATION

 REQUIRED FORMS/FEES & COLLECTION DATES

 The following forms and fees must be submitted completed in their entirety to Manual’s athletic office prior to the beginning of the student’s athletic participation (due dates listed at the bottom of this page). Students must be marked “CLEARED TO PARTICIPATE” on the following website 13-14 Paperwork/Fee Checklist for Athletic Participation.
 

       KHSAA Physical Form

o    physicals are valid for one calendar year from date of examination
o    must be signed as “cleared to participate” by physician
 
       JCPS Addendum to the Physical

o    must be signed by physician

        Parent / Student-Athlete Concussion Statement

o    Concussion Reference Card is available on school website under “Sports Forms”
 
       JCPS Parent Permission/Release Form

o    must be notarized
 
       JCPS Safety Video Signature Form

o    must be signed by parent and student

o    video is available on school website under “Sports Links”

       Transportation Waiver

o    must be notarized

       Parent Communication Agreement Form

       Social Networking Agreement

o    must be signed by parent and student

 
REQUIRED FEES
Each student-athlete is required to pay the insurance/trainers fees listed below to participate. Student-athletes pay this fee one time per school year. Make checks payable to Manual High School.

Student Information for Girls Basketball State Tournament Game on Thursday, March 14


GIRLS’ STATE BASKETBALL TOURNAMENT INFORMATION

 
GAME 1 – THURSday, March 14 @ 2:30 pm (EST)
 
Activity buses will be available for students wishing to attend Thursday’s game. Students that ride the activity bus to and from the game will be considered on a field trip. Students that use other means of transportation to and from the game will have to sign out of school through the attendance office (this will not be considered a field trip).
 
Students interested in riding the activity bus will need to purchase transportation and a game ticket, along with submitting a completed field trip permission slip (pick up in athletic office), in the bookstore during lunches on Monday, Tuesday and Wednesday (limited transportation/ticket spots may be available in the Attendance office on Thursday before 9:15 am – no guarantees). Students that wish to ride on the bus together must purchase tickets at the same time in order to be assigned to the same bus.
 
On Thursday, all students riding the buses will attend first lunch (we will not stop on the way to Bowling Green). We will begin loading buses at 11:10 am on Ray Baer Blvd. You will need to check-in with the bus chaperone prior to loading. The bus chaperone will request to see your bus and game tickets prior to boarding and will check you off the bus roster.
 
If you ride the bus down, you are required to ride the same bus back to Manual. Chaperones will have a roster to check-in riders when we leave the game. After the game, we will meet at the place where the bus dropped us off, outside gates 3 and 4. Please report promptly to that same bus for the return trip. We anticipate the buses returning to Manual by 7:00 pm, so please have your ride home scheduled. Students will receive one detention for every 15 minutes their ride is late arriving.
 
If we win on Thursday, and you want to attend Friday’s quarterfinal game, you will need to purchase Friday’s ticket immediately following Thursday’s game at the arena ticket booths for $9 (you will save $3 per ticket by purchasing in advance). Please remember to bring money for that purchase.
 
·   On Monday, March 11, Tuesday, March 12, & Wednesday, March 13, the following purchasing options will be available to students in the bookstore during all lunches:
o Thursday Game Ticket - $9
o Official T-Shirt - $10
o Thursday Bus Transportation - $14
o Thursday Bus Transportation, T-Shirt & Ticket Combo - $25
 
·   On Thursday, March 14, the following purchasing options will be available to students in the athletic office prior to 9:15 am.  
o Thursday Game Ticket - $9
o Official T-Shirt - $10
o Thursday Bus Transportation - $14 (limited availability)
o Thursday Bus Transportation, T-Shirt & Ticket - $25 (limited availability)

If you receive a RED MANUAL TICKET and need to exchange with Mr. Zuberer for a Friday game ticket, you need to meet him outside of Gate 3 at Diddle Arena at 2:00 pm.

Remember that we are representing Manual High School and Jefferson County Public Schools at the tournament. Please display good sportsmanship and have fun supporting our team. Since this is a school-sponsored activity, unacceptable behavior will be addressed according to the student code of conduct.
